Welcome to Allergy & Asthma Care and Prevention Center. To help your first visit be efficient please:

Fill out, before appointment, the New Patient Information Packet and bring them with you. Also, please be sure to bring your Insurance Card and your Co-pay amount that is due at tthe time of service. |

If your insurance requires a Referral to see a Specialist, it is your responsibility to see that it is received by us before your appointment. |

Have your medical records sent to our Center so that they arrive before your visit (Release Form enclosed upon request). |

Since it is possible that you may need allergy testing we ask that you avoid taking medicines that could interfere with allergy testing. See the "Instructions for Allergy Skin Testing" sheet. |
We look forward to both "care" and "prevention" of the symptoms that have prompted your evaluation. The initial visit could last 2 to 3 hours if skin testing is done. If you have any questions related to your initial visit, please do not hesitate to call our clinic nurse.
*NOTE: PLEASE ARRIVE 5-10 MINUTES EARLY FOR YOUR FIRST APPOINTMENT.
